I, [Driver Name], hereby authorize Xcel Multi Services Inc DBA Xcel Xpress and its authorized agents to obtain a copy of my motor vehicle driving record from any state Department of Motor Vehicles or licensing authority.

I understand this record will be used solely to evaluate my qualifications as a commercial motor vehicle operator pursuant to 49 CFR Part 391.23. I authorize the release of all information contained in my driving record, including but not limited to traffic violations, accidents, license suspensions, revocations, and cancellations.

This authorization is valid for the duration of my employment and for any subsequent annual MVR reviews required under DOT regulations. I hereby release all parties from any liability in connection with the furnishing of such information.

Pre-Employment Drug Test Consent

DOT Pre-Employment Drug & Alcohol Testing Consent
I, [Driver Name], understand and consent to submit to pre-employment controlled substances testing as mandated by the U.S. Department of Transportation (DOT) under 49 CFR Part 382 and the Federal Motor Carrier Safety Administration (FMCSA).

I understand that:
• A negative drug test result is required before I may operate a commercial motor vehicle on behalf of Xcel Multi Services Inc DBA Xcel Xpress.
• The test will screen for marijuana, cocaine, amphetamines, opioids, and phencyclidine (PCP).
• Refusal to submit to testing, or a confirmed positive result, will disqualify me from employment.
• My test results may be shared with the Drug and Alcohol Clearinghouse as required by 49 CFR Part 382.

I consent to the collection of a urine specimen and authorize the Medical Review Officer (MRO) to release my test results to Xcel Multi Services Inc DBA Xcel Xpress.
